i would like to get some information on my clients and i'm running a website that could do it. but 30% of my clients are from my blog. So i would like to know what html code I should use on my blog in order to get put up a form that will send all the info to my email.

The easiest way is to go to freedback.com. This is a free form tool for people without experience with MySQL. MySQL is required (using databases) to have your "own" form on your website.
